Our medical social workers accurately assess the social and emotional factors that may affect the patient and the family as a result of illness, disability and life-limiting conditions. They help patients and family members understand problems related to care at home and coordinate community resources and services
Specific services include:
- Explanation of health care resources, environmental risk factors and policies to patients, family and professional staff
- Assist with arranging services for the post-hospital patient needs at another facility or in home.
- Recognizing the social, cultural and religious variables which contribute to patient’s response to illness
- Provide patient advocacy through appropriate organizations
- Evaluation of program effectiveness for each patient and make adjustments if necessary
